How to Register

Registration will again be by mail this season.  Simply download and print-off the registration form (click here), include the documents listed below under "What to Mail-in to Register", and mail the package to:

South Fraser Field Lacrosse Association

Box #34141

17790 - 56 Avenue

Surrey BC  V3S 8C4

If you have any questions about registration, please e-mail our Registrar.

What to Mail in to Register

  • Completed Registration Form

  • Photocopy of Player's Birth Certificate

  • Player's Care Card number

  • Player's proof of residency (hydro or gas bill)

  • Cheque or money order made out to South Fraser Field Lacrosse Association (no cash or post-dated cheques accepted) for registration fee

  • Player's Assigned Jersey Number

Registrants must remember to fill-in all names, addresses, phone numbers (including cellular phone numbers) and e-mail addresses of all parents/guardians so that we can compile a comprehensive list of SFFLA voting members and so that we can send out important notices as they arise.

NOTE:  A player is not registered until a completed registration form is received, signed by parent or guardian, and payment is received in full (no post-dated cheques).

Players in each division will be entered on a wait list in the sequence of registration.  When enough players in a division register to create a team, a new waiting list will be formed in hopes that enough players register to complete another team.  If multiple teams are formed in a division the final team composition will be determined through tryouts.

Uniforms

Note that the registration fee does not include the purchase of jersey and shorts, which together cost approximately $95.00 and will be due and collected during tryouts.  Uniforms and the player's number belong to the player for their entire membership with SFFLA.  Specific number requests will be accommodated as best as possible, but alternatives should be considered by first-time registrants in case their preferred number is already taken.  Replacement jerseys and shorts can be ordered through the Team Manager as the player outgrows, loses or damages his/her existing jersey and/or shorts for a nominal fee.
